Shore Medical Center is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in ensuring the seamless delivery of patient care by coordinating with healthcare providers, patients, and families to achieve optimal health outcomes.
Key Responsibilities- Case Management: Develop and implement individualized care plans to address the unique needs of patients, including those with complex medical conditions.
- Communication: Facilitate effective communication among healthcare providers, patients, and families to ensure timely and accurate information exchange.
- Coordination: Coordinate patient care activities, including scheduling appointments, arranging transportation, and providing education and support to patients and families.
- Documentation: Maintain accurate and timely documentation of patient care activities, including progress notes, care plans, and discharge summaries.
- Quality Improvement: Participate in quality improvement initiatives to identify areas for improvement and implement changes to enhance patient care and outcomes.
- Education: Associate's degree or Bachelor's degree in a healthcare-related field, such as nursing, social work, or health administration.
- Experience: Minimum 2-3 years of experience in a healthcare setting, preferably in a case management or care coordination role.
- Skills: Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers. Proficiency in electronic health records and data analytics software.
- Certifications: Certified Case Manager (CCM) or Certified Care Coordinator (CCC) certification preferred.
