Job Description

The End of Life Planning Specialist reflects the mission, vision, and values of Northwestern Medicine. This role is responsible for providing information, education, guidance, and support to patients and families regarding Advance Directives.

Key Responsibilities:

  • Provide information, education, and guidance to patients and families regarding Advance Directives.
  • Engage patients and families in thinking about and planning for their goals in the context of a serious illness or life-limiting condition.
  • Identify and clarify patient's choices and preferences regarding life-sustaining treatment.
Requirements

This role requires a Bachelor's degree in Social Services and clinical experience emphasizing end-of-life discussions and goals of care. A Master's degree in Social Services and direct patient care experience in a medical setting are preferred.
