About the Role

CenterWell Home Health is seeking a skilled Physical Therapist to join our team. As a Home Health Physical Therapist, you will play a vital role in helping patients manage chronic conditions and recover from injuries or illnesses.

Key Responsibilities:

  • Plan and administer prescribed physical therapy treatment and training for patients with various injuries, illnesses, and functional disabilities.
  • Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
  • Develop and implement a conditioning and rehabilitation program consistent with the physician's Plan of Treatment and the overall goals of the patient/rehab team.
  • Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
  • Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides, supervise and evaluate their performance.
  • Accurately and thoroughly document patients' care observations, interventions, and evaluations.

Requirements:

  • Degree from an accredited Physical Therapy Program approved by the APTA.
  • Minimum of one year physical therapy experience preferred.
  • Home Health experience a plus.
  • Current and unrestricted Physical Therapy licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
