Job Overview

We are seeking an experienced Home Care Transition Coordinator to join our team at Ann Arundel Medical Center, Suburban & Sibley locations. This role involves coordinating home care services and resources for patients transitioning from hospital to home, acting as a discharge planning resource without providing hands-on clinical care.

Responsibilities
  • Coordinate home care services and resources for patients transitioning from hospital to home
  • Act as a discharge planning resource without providing hands-on clinical care
  • Collaborate with interdisciplinary teams to develop comprehensive care plans
  • Maintain accurate records and communicate effectively with patients, families, and healthcare providers
Requirements

To be successful in this role, you will need:

  • A graduate degree from an accredited nursing or allied health care professional program (BS preferred)
  • Licensure in a clinical discipline (RN, PT, OT) in the State of Maryland
  • Active CPR certification (Registered Nurse, Physical Therapist, Occupational Therapist)
  • Two years of experience in home care or home care discharge planning (preferred)
