Position Summary

The Service Manager 3 will play a vital role in supporting Level 3 day habilitation programs. This position involves assisting individuals who require daily intervention, supervision, and skills training in activities of daily living, both within the program setting and in the community. Participants may need moderate to significant physical assistance or accommodations. The Service Manager will receive specialized training to provide enhanced supervision and interventions tailored to address multiple disabilities, including behavioral supports. Responsibilities include developing, implementing, and overseeing individualized training programs, managing cases for designated individuals, and fulfilling general duties as needed.

Work Schedule

Standard hours are from 8 AM to 4 PM, Monday through Friday.

Key Responsibilities
  1. Deliver services to individuals in accordance with DDS, DMH, MassHealth, HUD, DPH, CARF, and all relevant regulatory standards, alongside Bridgewell's policies and procedures.
  2. Conduct and interpret assessments to evaluate each individual's skill level annually.
  3. Design and implement personalized programs based on individual needs, following their Day Habilitation Support Plan.
  4. Provide training in designated areas to individuals and small groups.
  5. Create adaptive equipment and tools to assist individuals in achieving their goals.
  6. Oversee the execution of the Day Habilitation Support Plan and maintain individual records in TIER.
  7. Support individuals in making informed decisions regarding their daily routines and activities.
  8. Ensure individuals have opportunities for communication with family, friends, and significant others.
  9. Facilitate participation in community events, including volunteer opportunities and local activities of interest.
  10. Maintain necessary certifications, including FA/CPR and PABC.
  11. Perform additional duties as assigned.
Qualifications

Education and Experience:

  1. Associate's Degree in Human Services or equivalent experience.
  2. At least one year of experience working with individuals with developmental or psychiatric disabilities.
  3. Training and/or experience in areas such as autism and behavioral support is preferred.

Skills and Knowledge:

  1. Understanding of normalization principles, behavior management, and human rights.
  2. Strong written and verbal communication skills for clear documentation.
  3. Excellent organizational and problem-solving abilities.
  4. Valid driver's license with a minimum of one year of driving experience.
  5. Proficiency in Microsoft products and web-based applications supported by the agency.
Physical Requirements
  1. Frequent stair climbing is required.
  2. Ability to lift up to 50 pounds.
  3. Must be able to drive a vehicle and meet agency driving record standards.
  4. Normal eyesight range with or without corrective lenses.
  5. Normal hearing range with or without hearing aids.
