Responsibilities:- Provide comprehensive nursing care to patients in their homes, as prescribed by physicians in the plan of care.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Manage nursing treatments and activities to meet the unique needs of each patient.
- Evaluate patient progress and utilization of services, and communicate with the care team, patient, family members, physician, and case manager as needed.
- Geriatric Home Health experience
- Texas Nursing License
- Strong organizational and self-management skills
- Strong and compassionate customer service skills
- Valid driver's license
