Grace Healthcare Services is seeking a compassionate and skilled Social Worker to join our team. As a Hospice Social Worker, you will provide direct casework and counseling services to patients and their families, ensuring their physical, emotional, and spiritual needs are met.

**Job Summary:**

We are looking for a dedicated and empathetic individual to provide psychosocial assessments, develop care plans, and facilitate referrals to community resources. You will work collaboratively with our Interdisciplinary Team to ensure continuity and comprehensiveness of care.

**Responsibilities:**

* Provide psychosocial assessments and develop care plans in collaboration with the Interdisciplinary Team
* Facilitate referrals to community resources and support services
* Participate in Interdisciplinary Team meetings and staff meetings
* Maintain accurate and timely clinical records
* Provide counseling and support to patients and their families
* Collaborate with healthcare professionals to ensure comprehensive care

**Requirements:**

* Master's degree in Social Work or related field
* Licensed Social Worker
* Strong interpersonal and communication skills
* Experience in healthcare and clinical social work
* Knowledge of psychosocial factors operative in terminally ill patients and families
* Reliable transportation and current state driver's license
