Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team at Hospice of Michigan. As a key member of our interdisciplinary team, you will provide high-quality patient care to individuals and their families in need of end-of-life support.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs
  • Develop and implement individualized plans of care, collaborating with the interdisciplinary team to prioritize patient and family-oriented services
  • Provide end-of-life care, including comfort care and symptom management, according to NorthStar Care Community indicators
  • Support and educate patients and families regarding disease processes, self-care techniques, and end-of-life care
  • Coordinate durable medical equipment and supplies as needed
  • Participate in quality assurance processes, internal/external education/training, marketing, and other activities promoting the organization's mission
Requirements
  • Graduation from an accredited school of nursing with a current license to practice as a registered professional nurse in the State of Michigan
  • Minimum of one to two years of varied nursing experience, with hospice experience and CHPN certification preferred
  • Ability to use technology (hardware and software) required of clinicians in their practice
  • Excellent interpersonal skills, sound judgment, effective organizational, prioritization, and follow-through skills, attention to detail, tact, dependability, emotional intelligence, and the ability to maintain confidentiality and promote positive, constructive relationships
