Overview

Responsibilities

As a Social Worker on our Home Health and Hospice team, you'll be responsible for:

  • Assessing the psychosocial status of clients and communicating findings to the registered nurse
  • Carrying out social evaluations and planning interventions based on evaluation findings
  • Maintaining clinical records on all clients referred to social work
  • Providing information and referral services to clients and families/caregivers regarding environmental needs and resources
  • Marketing and promoting agency programs and events in the local community
  • Serving as a liaison between clients or families/caregivers and community agencies
  • Maintaining collaborative relationships with organization personnel to support client care
  • Participating in the development of the total plan of care and case conferences as required
Qualifications

To be successful in this role, you'll need:

  • Bachelor's degree in Social Work
  • Licensed as a Social Worker in Minnesota and North Dakota
  • One year experience in healthcare
  • Excellent verbal, written communication skills and organizational skills
  • Basic computer skills
