  • Responsibilities include assessing nutrition status, implementing interventions, and collaborating with interdisciplinary teams.

Key Responsibilities:

  • Reviews participant nutritional needs prior to enrollment into the program.
  • Completes assessments and develops personalized plans of care.
  • Counsels participants and caregivers on healthy eating habits and meal planning.
  • Collaborates with healthcare professionals to ensure comprehensive care.
  • Maintains accurate records and communicates effectively with team members.

Requirements:

  • A Bachelor's Degree in Nutrition or related field.
  • Completion of a dietetic internship or equivalent practicum.
  • Registration as a Dietitian by the Commission on Dietetic Registration or eligibility for consideration.
  • At least one year of experience working with aging adults.
  • Strong analytical skills and ability to communicate effectively.
