Home Health Care Coordinator

We are seeking a skilled Home Health Care Coordinator to join our team at All Care Group of Home Health Providers. As a dedicated healthcare professional, you will play a vital role in ensuring the highest level of care for our patients.
Responsibilities:- Coordinate nursing and physical therapy staff schedules to match patient needs with competent staff, prioritizing and adjusting schedules as needed.
- Monitor and control schedule changes, ensuring seamless service delivery.
- Confirm patient demographics through phone calls, guaranteeing accurate information for efficient care.
- Pre-schedule and admit patients into our system, streamlining the onboarding process.
- Reroute canceled appointments based on Care Coordinator instructions, minimizing disruptions.
- Ensure timely services for patients, maintaining a high level of satisfaction.
- Demonstrate exceptional organizational skills, managing multiple tasks with precision.
- Type accurately and efficiently, processing information with speed and accuracy.
- Perform other duties as assigned by Care Coordinators and the Director of Patient Care and Services (DPCS).
- Proficiency in handling heavy telephone use, navigating complex communication scenarios.
- Ability to thrive in a fast-paced environment, delivering results under pressure.
- Positive attitude, excellent people skills, and a strong desire to provide top-notch customer service.
- Independence and self-motivation, working effectively without constant supervision.
- Reliability and dependability, adhering to deadlines and commitments.
- Proficiency in computer applications, specifically MS Office, to support administrative tasks.
- Able to multitask, juggle multiple priorities, and adapt to changing situations.
- A goal-oriented individual, productive, organized, patient, and enthusiastic about interacting with patients and colleagues.
- Problem-solving skills, identifying and resolving issues efficiently and effectively.
- Flexibility with time, occasional adjustments to accommodate dynamic scheduling needs.
- College or High School diploma or equivalent, demonstrating a solid educational foundation.
- Minimum typing speed of 45-50 wpm, ensuring accurate and efficient data entry.
- Familiarity with medical terminology, a plus for understanding complex healthcare concepts.
- Strong computer skills, including knowledge of MS Office applications, to support administrative tasks.
- A willingness to grow with our company, embracing opportunities for professional development.
$45,000 - $55,000 per year, depending on experience and qualifications.
