Transplant Nurse Coordinator Post Liver

We are seeking a highly skilled and compassionate Transplant Nurse Coordinator to join our team at MedStar Health. As a key member of our transplant team, you will play a vital role in ensuring the smooth transition of patients through the transplant process.
Responsibilities:- Coordinate the care of transplant patients, including pre-transplant evaluation, post-transplant follow-up, and ongoing management of complications.
- Collaborate with the transplant team, including surgeons, hepatologists, nephrologists, and other healthcare professionals to ensure seamless care.
- Develop and implement individualized care plans for patients, taking into account their unique needs and circumstances.
- Provide education and support to patients and their families, addressing their concerns and promoting optimal outcomes.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Current RN licensure in the District of Columbia.
- 3-4 years of experience in transplant nursing or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Knowledge of transplant nursing principles and practices, including immunosuppression management and post-transplant care.
