Job Summary:
Creative Health Solutions, LLC is seeking a skilled and passionate Physical Therapist to join our team. As a Physical Therapist, you will work closely with a team of healthcare professionals to develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.
Key Responsibilities:
- Conduct thorough functional assessments of patients to identify areas of improvement and develop individualized treatment plans.
- Develop and implement therapy programs to improve patients' physical function, mobility, and overall quality of life.
- Collaborate with nursing staff, patients, and families to ensure effective care delivery and provide education on exercises and therapeutic procedures.
- Communicate with patients and families to ensure understanding of treatment plans and progress.
- Supervise Assistants as needed and provide guidance on therapy procedures.
- Stay up-to-date with current knowledge of community resources and refer patients as needed.
Requirements:
- Master of Science degree in Physical Therapy or Bachelor of Science degree in Physical Therapy and five years of direct practice experience.
- Current license or eligible for license under statutes in the state of practice.
- Excellent oral and written communication and interpersonal skills.
- Adequate computer skills to complete required job duties and computerized documentation.
- Demonstrate ability to assess patient needs and develop and implement a comprehensive plan of care.
- Strong organizational skills to effectively maintain multiple projects simultaneously.
- Requires creativity and ability to work independently.
