Join Our Team as an MDS Nurse

Signature HealthCARE is a family-based healthcare company that offers integrated services in 7 states across the continuum of care. We are seeking compassionate, detail-oriented, and driven nurses who are eager to make a difference in the lives of our residents.

Program Overview

Our 8-week training program is designed to equip Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) with the skills and knowledge necessary to excel in long-term care. The program includes classroom and facility-level training, covering all aspects of the MDS process, including resident assessments, care planning, documentation, and compliance with federal and state regulations.

Key Responsibilities
  • Conduct resident assessments to capture the care needs of residents
  • Develop individualized care plans based on assessment data to ensure comprehensive care
  • Gain a deep understanding of MDS regulations and guidelines to maintain compliance with federal and state requirements
  • Work closely with other healthcare professionals to ensure holistic resident care
  • Master the art of precise and timely documentation to support care delivery and regulatory compliance
Program Outcomes
  • Assess patients accurately and evaluate the quality of care for existing residents
  • Effectively collect, monitor, and document patient care information
  • Create and implement individualized care plans that include both short-term and long-term goals for resident improvement
  • Collaborate with caretakers, families, and multidisciplinary treatment teams to provide the highest quality of care for residents
What You Need to Make a Difference
  • Must hold a current and active LPN or RN license in the state of employment
  • Strong organizational skills and attention to detail are crucial for accurate MDS assessments and documentation
  • Must be able to provide excellent service to residents, families, and colleagues
  • Effective communication with residents, families, and the interdisciplinary team is essential
  • Willingness to engage fully in the 8-week training program and apply new knowledge in practice
