Job Description:

We are seeking a highly skilled Neighborhood Health Practitioner to join our team at Cook Children's. As a Neighborhood Health Practitioner, you will be responsible for providing high-quality patient care in a collaborative practice setting with physicians. You will perform patient assessment, diagnosis, and intervention according to approved protocols, while also evaluating and incorporating social determinants of health into treatment plans. The salary for this position is estimated to be around $130,000 - $160,000 per year, depending on experience, along with a comprehensive benefits package and opportunities for professional growth and development.

About the Role:

The successful candidate will have a Master's Degree in Nursing or Physician Assistant Studies, and current licensure and certification as required by the state of Texas. They will also possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. Our ideal candidate will have experience working with diverse patient populations and be able to provide culturally sensitive care. Some of the key responsibilities of this role include:
