Home Health Clinical Director

We are seeking a highly skilled and experienced Home Health Clinical Manager to join our team at Choice Health at Home. As a key member of our leadership team, you will be responsible for ensuring the quality of patient care in the home and overseeing the administration of our clinical departments.
Key Responsibilities- Direct and coordinate clinical departments to ensure continuity, quality, and safety of services delivered in compliance with state and federal regulations.
- Promote the philosophy and administrative policies of the Agency, ensuring quality of patient care and compliance with regulatory requirements.
- Supervise and provide direction to subordinates, ensuring quality, compliance with the Plan of Care, assessment and reassessment of patients' needs, and continuity of services by appropriate health care personnel.
- Monitor the QAPI Program and ensure appropriate corrective measures are performed.
- Promote compliance with all fiscal intermediary and/or other third party payors, through education, coaching, and other assistance as necessary.
- Participate as a member of the Interdisciplinary Group (IDG), assist in development of the IDG plan, and participate in QAPI activities.
- Monitor systems and identify problem areas to the Administrator both verbally and through written reports.
- Meet mandatory continuing education requirements of the agency and licensing board.
- Promote and educate regarding the concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
- Use effective interpersonal relations and communication skills; facilitate the use of these skills by other team members to achieve desirable outcomes.
- Current RN License
- Valid Driver's License
- Valid Auto Insurance
- CPR Certification
- At least 2 years of Home Health management experience
- HomeCare HomeBase experience preferred
