About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Simpsonville Post Acute. As an LPN, you will play a vital role in providing direct care to our residents, ensuring their care plans are carried out appropriately.

Responsibilities
  • Provide direct care to residents, including administering medications, wound care, and vital sign monitoring.
  • Supervise nursing assistants to ensure compliance with state, federal, and local standards.
  • Participate in interdisciplinary team meetings to discuss resident care and develop care plans.
  • Collaborate with other healthcare professionals to ensure seamless care transitions.
