Job Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care for patients served by the Ryan White Program. This position assists physicians and Advanced Practice Providers (APPs) in identifying, monitoring, and coordinating service needs. The LPN will participate in quality assurance initiatives and perform other duties as assigned.

Responsibilities
  • Attend clinic and provide skilled nursing care based on patient needs. Evaluate and adjust plans of care as necessary to achieve desired outcomes. Record vital signs, medication history, social history, and triage patients. Clean and restock patient rooms. Educate patients and monitor their compliance with treatment regimens.
  • Provide coordination of care for service providers to meet clinical needs. Identify client needs, assess the status of functional, psychological, social, environmental, financial, and other support systems as factors in the development of goals and care plans.
  • Accurately and timely document information to support medical and administrative review.
  • Participate in quality assurance review and assessment as requested and/or mandated by federal or institutional directives.
  • Participate in other grant initiatives as requested and perform all other job-related duties as assigned.
Requirements

High School diploma, GED, or equivalent from a recognized State or Federal accrediting organization. Graduate from an accredited LPN program and licensed as a LPN in the state of Georgia or Compact license to include Georgia. Current American Heart Association BLS certification.

Knowledge, Skills, and Abilities

Ability to maintain confidentiality. Excellent interpersonal, written, and verbal communication skills. Detail-oriented with strong prioritization and organizational skills. Proficient in Microsoft Office and other computer software/databases.

Shift, Salary, and Benefits

Shift: Days; Monday - Friday. Pay Band: B8. Salary: $21.83 - $24.03. Salary to be commensurate with qualifications of the selected candidate within the established range (generally minimum-midpoint) of the position. Recruitment Period: 4/24/24 - Until Filled.
