We are seeking a skilled Physical Therapist to join our team at AccentCare, where you will have the opportunity to make a meaningful impact on the lives of our patients. As a Home Health Physical Therapist, you will be responsible for providing high-quality patient care in a home-based setting.

Key Responsibilities
  • Conduct thorough assessments and develop personalized treatment plans to meet the unique needs of each patient
  • Provide one-on-one therapy sessions to patients in their homes, promoting independence and improving functional abilities
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication
  • Document patient progress and outcomes, utilizing data to inform treatment decisions and drive quality improvement
Requirements
  • Master's degree in physical therapy from an accredited program
  • Current licensure to practice as a physical therapist in the state of operation
  • Minimum 9 months of experience as a physical therapist, with a preference for community or home health experience
  • Valid CPR certification and a reliable vehicle for travel to patient locations
