Job Summary:

As a Licensed Practical Nurse, you will deliver direct patient care, assess resident needs, and collaborate with our interdisciplinary team to ensure quality care. You will also supervise Resident Care Assistants, coordinate care plans, and participate in education and training initiatives.

Responsibilities:

  1. Deliver individualized nursing care to residents, promoting independence and dignity
  2. Assess resident health status, documenting changes and concerns
  3. Communicate effectively with residents, family members, and healthcare professionals
  4. Supervise and train Resident Care Assistants, ensuring they meet care standards
  5. Mentor and guide Lead Resident Care Assistants in their roles
  6. Coordinate care with community providers through effective case management processes
  7. Monitor and report any signs of elder abuse, neglect, or exploitation
  8. Respond promptly to emergencies and call for backup when necessary

Requirements:

  • Active LPN licensure in Connecticut
  • CPR certification
  • One to two years of nursing experience preferred
  • Experience working with seniors in a healthcare setting desired
