DaVita Inc. is seeking a dedicated Full-Time Senior Nurse Practitioner Manager to become a vital part of our Integrated Kidney Care team.
This position offers a Hybrid REMOTE opportunity, allowing for work from home with occasional regional travel.
In this capacity, you will deliver clinical leadership, oversight, coaching, and onboarding for Nurse Practitioners, ensuring the provision of quality and cost-effective care for patients with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD).
This role operates independently while collaborating with operational and clinical leadership on budgeting, quality initiatives, outcomes, and performance metrics.
Regular interaction with nephrology partners is expected.KEY RESPONSIBILITIES
- Act as the clinical lead for a team of Nurse Practitioners, overseeing their activities. Ensure NP quality and productivity, including accurate documentation, appropriate diagnoses, and closing care plan gaps. Support NP recruitment and manage the onboarding process for new team members. Provide continuous clinical guidance and training to enhance understanding of CKD, ESKD, and related chronic conditions.
- Collaborate with the Medical Officer and Operations leadership to drive clinical and operational strategies. Identify opportunities to optimize NP workflows and focus management efforts on high-risk, high-utilization patients.
- Lead complex case reviews and provide timely support for escalated cases involving high-utilization and high-risk patients, working alongside specialized resources and the patient's care team.
- Adhere to DaVita policies and procedures in compliance with State and Federal regulations for patient care and services, including CMS, HIPAA, OSHA, and other governing bodies; maintain standards necessary for organizational accreditation.
- Perform additional duties as assigned.
QUALIFICATIONS
- 8-10 years of experience as a Nurse Practitioner; background in population health, value-based care, or primary/family care is preferred.
- Minimum of 2 years of supervisory or leadership experience as a Nurse Practitioner.
- Master’s degree in Nursing.
- Exceptional verbal and written communication skills.
- Proven proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation related to chronic disease management.
- Ability to handle confidential information with discretion and sound judgment.
- Experience with documentation and coding from a Medicare Advantage risk perspective and conducting Health Risk Assessments for high-risk/chronically ill patients.
- Intermediate computer skills and proficiency in MS Word, Excel, and Outlook.
- Strong organizational skills and attention to detail.
