Home Health Speech Language Pathologist

We are seeking a skilled Speech Language Pathologist to join our team at VNA Health Group. The ideal candidate will have a Master's degree in Speech-Language Pathology and be licensed with CCC-SLP certification. Prior clinical experience and community-based experience are preferred. The Speech Language Pathologist will be responsible for providing speech and language therapy evaluation and treatment to patients in their homes or alternate community settings. The position requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team. The Speech Language Pathologist will also be responsible for assessing and evaluating clients for speech and language therapy needs, developing and implementing appropriate plans of action, and documenting all changes in client condition and updating care plans and outcomes accordingly. The ideal candidate will be able to work in a fast-paced environment and be able to adapt to changing situations. Responsibilities:

  • Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
  • Assesses and evaluates client for speech and language therapy needs and develops and implements appropriate plan of action in collaboration with the physician.
  • Documents all changes in client's condition and updates care plans and outcomes accordingly.
  • Identifies counseling needs and plans appropriate teaching strategies.
  • Monitors quality and appropriateness of services provided.
  • Adheres to agency infection control and safety policies.
  • Adheres to the organization's policy in regards to absenteeism and appearance.

Qualifications:

  • Licensed Speech-Language pathologist with CCC-SLP certification and Master's degree in Speech-Language Pathology.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills.
  • Strong communication and interpersonal skills.
  • Valid NJ drivers license.

Working Conditions/Physical Demand :

  • Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation.
