This role requires a current, unrestricted RN license in New Jersey, 2+ years of RN experience in a hospital setting, acute care, direct care experience, and/or experience as a Case Manager. You'll also need 1+ years of paid professional experience working directly with the elderly or physically disabled in an institutional or community setting. Ability to navigate a Windows environment, utilize Outlook, and create, edit, save, and send documents using Microsoft Word and Excel is also essential.

As a Telephone Case Manager RN, you'll be responsible for making outbound calls to assess members' current health status, demonstrating an ability to deliver effective telephonic case management, identifying gaps or barriers in treatment plans, providing patient education, interacting with Medical Directors, coordinating care for members, making referrals to outside sources, coordinating services, educating members on disease processes, encouraging members to make healthy lifestyle changes, and documenting and tracking findings.

The hourly range for this role is $28.03 to $54.95 per hour, based on several factors including education, work experience, certifications, and more.
