Job SummaryWe are seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Hospice Care Specialist. As a key member of our care team, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.
Responsibilities- Perform initial nursing assessments on new admissions to identify patient needs and develop individualized care plans.
- Collaborate with healthcare professionals to coordinate patient care and ensure continuity of services.
- Communicate patient progress to the clinical manager, physician, and care team to ensure seamless care delivery.
- Develop, prepare, and maintain accurate and timely patient care progress records in accordance with care center policies.
- Record pain and symptom management changes and outcomes as necessary.
- Educate families on patient condition, needs, and care to ensure informed decision-making.
- Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Current RN license, specific to the state(s) you are assigned to work.
- Three years of nursing experience preferred.
- Current CPR certification.
- HCHB experience is a plus.
- Weekend availability to cover Brevard County Territory.
- Valid driver's license, reliable transportation, and liability insurance.
