Job Summary

We are seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Hospice Care Specialist. As a key member of our care team, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.

Responsibilities
  • Perform initial nursing assessments on new admissions to identify patient needs and develop individualized care plans.
  • Collaborate with healthcare professionals to coordinate patient care and ensure continuity of services.
  • Communicate patient progress to the clinical manager, physician, and care team to ensure seamless care delivery.
  • Develop, prepare, and maintain accurate and timely patient care progress records in accordance with care center policies.
  • Record pain and symptom management changes and outcomes as necessary.
  • Educate families on patient condition, needs, and care to ensure informed decision-making.
  • Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
  • Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Requirements
  • Current RN license, specific to the state(s) you are assigned to work.
  • Three years of nursing experience preferred.
  • Current CPR certification.
  • HCHB experience is a plus.
  • Weekend availability to cover Brevard County Territory.
  • Valid driver's license, reliable transportation, and liability insurance.
