Job Summary

We are seeking a dynamic and compassionate Registered Nurse to join our team as a Home Health Nurse Specialist. As a key member of our Home Health Division, you will provide high-quality care and service to our patients, working collaboratively with our interdisciplinary team to ensure seamless care coordination.

Responsibilities

  • Provide skilled nursing care to patients in their homes, including wound care, ventilator management, and lab draws.
  • Conduct thorough assessments of patients' needs and develop personalized care plans to meet their unique requirements.
  • Collaborate with patients, families, and caregivers to educate them on disease management, medication adherence, and self-care techniques.
  • Communicate effectively with the Home Health Team, including physicians, therapists, and social workers, to ensure comprehensive care coordination.
  • Document patient progress, including assessments, interventions, and outcomes, in accordance with agency policies and regulatory requirements.

Requirements

  • Current Michigan Registered Nurse (RN) licensure.
  • Minimum one (1) year of home health nursing experience preferred.
  • Demonstrated knowledge and skills to provide care to and communicate with all ages of the population.
  • Updated Tuberculin (TB) test and current CPR certification required.
  • Clear Criminal History/Drug Screen.
  • Compassionate and caring demeanor, with a professional presentation at all times.
  • Ability to travel within the service area of AdvisaCare (approximately a 30-mile radius from your home).
