Centra Healthcare Allied is seeking a highly skilled Physical Therapist to join their team in Centerville, Tennessee. As a Physical Therapist - Home Health Care Specialist, you will be responsible for providing expert care to patients in the comfort of their own homes.
About the RoleThis contract position involves working 5x8 days, with a focus on delivering high-quality physical therapy services to patients in need. The ideal candidate will possess at least one year of recent experience as a Physical Therapist specializing in Home Health Care and be licensed to practice in the state of Tennessee.
Key Responsibilities- Deliver personalized physical therapy services to patients in their homes
- Develop and implement effective treatment plans to improve patient outcomes
- Collaborate with healthcare professionals to ensure seamless care coordination
- Maintain accurate records and communicate effectively with patients and families
As a valued member of the Centra Healthcare Allied team, you can expect:
- A competitive salary range of $85,000 - $115,000 per annum
