Occupational Therapist Job Description

At Grace and Mercy Home Care Services, LLC, we are seeking a highly skilled and compassionate Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in helping patients recover from illness, injury, or disability, and achieve their full potential.

Responsibilities:
  • Develop and implement individualized treatment plans to promote patient health, independence, and well-being.
  • Conduct thorough assessments of patients' physical and mental abilities, and analyze medical data to determine realistic rehabilitation goals.
  • Plan and conduct occupational therapy sessions to help patients learn work and life-management skills, and adapt to their limitations.
  • Collaborate with the rehabilitation team to select activity programs and coordinate occupational therapy with other therapeutic activities.
  • Evaluate patients' progress, and prepare reports that detail their progress and recommendations for future treatment.
  • Provide training and supervision in therapy techniques and objectives for Home Health Aides (HHAs) to assist patients when needed.
  • Recommend special supplies and equipment, such as splints, braces, and computer-aided adaptive equipment.
  • Participate in departmental and general meetings as needed.
Qualifications:
  • Must possess a New York state Occupational License.
  • Must be a graduate of a program in occupational therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association, or a graduate of a curriculum in occupational therapy recognized by the World Federation of Occupational Therapists and eligible for registration with the American Occupational Therapy Association.
  • Must be registered with the American Occupational Therapy Association.
Preferred Skills:
  • Ability to relate appropriately to, communicate effectively with, and develop trusting relationships with diverse patient populations and with staff.
  • Ability to meet or exceed the Service Excellence Standards of the New York State Department of Health.
  • Ability to work on an as-needed basis to meet operational needs of the agency.
  • Bi-lingual in Spanish/English is a plus.
