Job Description

Job Summary:

The Registered Nurse Case Manager (RNCM) at PACE Southeast Michigan is a key member of our interdisciplinary team, responsible for providing high-quality, patient-centered care to our frail elderly participants. As a leader in our organization, the RNCM will utilize a systematic approach to nursing practice, incorporating assessment, planning, implementation, and evaluation of care to achieve optimal outcomes.

Key Responsibilities:

  • Assessment and Planning: Conduct thorough assessments of participants' needs, develop and implement individualized care plans, and collaborate with the interdisciplinary team to ensure seamless care coordination.
  • Care Coordination: Lead and direct licensed and non-professional nursing staff in the delivery of care, ensuring timely and effective implementation of care plans.
  • Education and Training: Provide education and training to participants, caregivers, and family members on nursing care needs and other related topics, utilizing adult learning principles to promote effective learning.
  • Medication Management: Administer medications and treatments as ordered by physicians or NPs, monitor participants' responses, and notify medical personnel of any changes in status.
  • Quality Improvement: Participate in quality assurance projects, review current literature, and implement evidence-based practices to ensure high-quality care.
  • Leadership and Collaboration: Lead and direct other licensed and non-professional nursing staff, collaborate with the interdisciplinary team, and participate in the development and implementation of organizational policies and procedures.
  • Documentation and Communication: Maintain accurate and timely records of participants' functional and health status, progress toward care plan outcomes, and revisions to care plans.
  • Advocacy and Customer Service: Advocate on behalf of participants, demonstrate accountability in resolving concerns or issues, and promote high levels of participant satisfaction.

Requirements:

  • Education and Experience: Bachelor's degree in Nursing (BSN) preferred, current Michigan licensure as a Registered Nurse, and at least one year of experience working with a frail or elderly population.
  • Skills and Abilities: Strong leadership and communication skills, ability to establish and maintain interpersonal and interdepartmental relationships, and proficiency in adult learning principles.
  • Physical Demands: Frequent walking, bending, and lifting of 40 pounds or more may be required in the performance of duties.

Working Conditions:

  • Works in participants' homes, which may be uncontrolled environments.
  • May be exposed to potentially infectious materials, blood-borne disease pathogens, and hazardous waste.
  • Must be medically cleared for communicable diseases and have all immunizations up-to-date after engaging in direct participant contact.
  • Driving is required within the PACE Southeast Michigan catchment area, with possible exposure to extreme temperatures.
