Key Responsibilities- Deliver Skilled Nursing Care: Provide one-to-one patient care in a home environment, executing physicians' prescribed plans of care and documenting all activities, assessments, and nursing actions in a timely manner.
- Assess and Coordinate Care: Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
- Develop and Implement Care Plans: Develop, implement, and update nursing care plans to achieve expected outcomes and promote patient well-being.
- Collaborate with Healthcare Professionals: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.
- Provide Education and Support: Educate patients and families on disease management, self-care techniques, and prevention strategies to promote healthy outcomes.
- Education and Licensure: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing; current, unrestricted state license as a Registered Nurse in the state of practice.
- Certifications and Experience: Current CPR certification; demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures; at least 6 months of recent experience as a Licensed Nurse in a clinical care setting.
- Skills and Abilities: Strong attention to detail, effective problem-solving and conflict resolution, good organization and communication skills, and ability to work independently and as part of a team.
