Job Summary

We are seeking an Independent Optometrist to lease space in our Walmart stores. As an independent lease holder, you will have the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule. You will also have the opportunity to opt into equipment, instrumentation, administrative, and/or opt-tech support services for a fair market value.

Key Responsibilities
  • Provide eye and vision care services to patients in a Walmart store setting
  • Establish and maintain a patient schedule
  • Set and manage your own fee schedule
  • Have the opportunity to opt into equipment, instrumentation, administrative, and/or opt-tech support services
Requirements
  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school
  • Valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
