The Oncology Nurse Navigator at Inova Health System plays a vital role in ensuring seamless care for patients with cancer. This dynamic professional will work closely with patients, families, and healthcare teams to navigate the complex healthcare system, providing emotional support, clinical education, and expert guidance to promote informed decision-making and optimal outcomes.
Key Responsibilities- Coordinates patient care plans, including answering questions, scheduling appointments, and providing community resources.
- Provides educational information and advocacy to patients throughout the care process, including survivorship.
- Functions as a liaison to internal and external community services, referring cases to appropriate resources as needed.
- Creates a seamless flow through the health system to facilitate patient care.
- Participates in coordination of the plan of care with the multidisciplinary team, promoting timely follow-up on treatment and supportive care recommendations.
- 3-5 years' progressive clinical nursing experience with primary care management of patient population.
- BSN degree.
- Current Basic Life Support (BLS) certification.
- Certification in oncology within 2 years of employment.
