**Responsibilities:**

* Under the direction of the RN/Mid-level Provider/Physician, contribute to the assessment, planning, implementation, and evaluation of patient care
* Collect and monitor assessment data, reporting physiological, psychological, sociocultural, and spiritual needs of patients and families
* Provide direct patient care under the direction of the RN/Mid-level Provider/Physician, consistent with policies and procedures and unit-based routines
* Inform patients and families about treatments and medications being received, equipment being utilized, and other related activities
* Document care provided via computer/written patient record, in a manner consistent with guidelines
* Greet and communicate with patients and families in a manner consistent with caring and respect
* Communicate and collaborate with other members of the health care team to ensure continuity of care and coordination of service
* Clean patient's environment/work area/equipment and communicate any equipment malfunctions to appropriate department/unit personnel
* Document patient chargeable items according to standards
* Participate in selected quality monitoring activities under the direction of the RN/Mid-level Provider/Physician
* Demonstrate a high standard of moral and ethical behavior
* Recognize appropriate and ethical behaviors and identify resources for resolution as needed
* Advocate for privacy, confidentiality, and security of patient, staff, and organization data

**Requirements:**

* High School Diploma or GED
* Attendance is mandatory for the first 90 days
* Required certifications: MI LPN, MI Fingerprints, and AHA BLS
* Must have at least 1 year of LPN experience
* Associate's Degree, Graduate of an approved nursing program, Successful completion of competency-based orientation program

**Preferred Qualifications:**

* Continuing Care Neuro Residential 2 Yrs preferred
