Job Summary

We are seeking a highly skilled and compassionate Healthcare Coordinator to join our team at Trinity Health. As a key member of our Health Home Care Management Team, you will play a vital role in coordinating and collaborating with community providers to ensure positive outcomes for our members.

Key Responsibilities
  • Develop and Implement Care Plans: Work closely with members and their families to develop comprehensive care plans that address their medical and behavioral health needs, social determinants of health, and activities of daily living.
  • Coordinate Services: Collaborate with an interdisciplinary care team to coordinate and integrate a comprehensive array of services, including care coordination, advocacy, information/education, and referrals to community resources and providers.
  • Conduct Assessments: Conduct comprehensive assessments to identify members' needs and develop person-centered care plans that promote optimal wellness and recovery.
  • Communicate Effectively: Develop and maintain strong relationships with members, community providers, and other stakeholders to ensure effective communication and collaboration.
  • Meet Regulatory Requirements: Adhere to policies and procedures as developed by the lead health home and NYS DOH.
Requirements
  • Education and Experience: Bachelor's Degree in Human Services or related field required; two years or more working with individuals with chronic medical conditions and behavioral health issues preferred.
  • Skills and Abilities: Extensive planning, organizational skills, and excellent communication skills; ability to work independently and as part of a team; ability to drive and possess a valid driver's license and automobile insurance.
