Home Health Physical Therapist Opportunity

We are seeking a skilled Physical Therapist to join our team at UP Health System Home Care & Hospice, covering the Iron Mountain area. As a Physical Therapist, you will play a vital role in helping patients achieve their maximum potential and improve their overall well-being.

Key Responsibilities:
  • Evaluate patients' functional status and physical therapy needs, working closely with physicians to develop effective treatment plans.
  • Deliver high-quality physical therapy interventions to relieve pain, restore function, and maintain maximum functioning.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Monitor patients' progress, reporting any changes or concerns to supervising nurses and physicians.
Requirements:
  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification.
  • Valid driver's license and vehicle insurance.
