We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at Lifeline Health Care of Pulaski. As an LPN, you will provide high-quality patient care and perform technical skilled care in the patient's home, working in collaboration with our interdisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities:
  • Provide services within the scope of practice as defined by state laws governing nursing, in accordance with the plan of care, and in coordination with other healthcare team members.
  • Instruct patients and their families in care and maintenance of a healthy environment for the patient.
  • Observe, record, and report patient responses to treatment and changes in condition to supervising nurses and/or physicians daily.
Requirements:
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
