Job SummaryWe are seeking a highly skilled Nurse Practitioner to join our team in Central Phoenix, AZ. The successful candidate will provide home-based care to geriatric, frail, and clinically complex patients. The ideal candidate will have experience in home-based care, geriatrics, or palliative care and a strong ability to build relationships with patients and their families.
Key Responsibilities- Complete thorough initial visits with patients, including medical, social, and behavioral assessments.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Collaborate with the multidisciplinary care team to ensure seamless care coordination and communication.
- Participate in team huddles to review care plans and assess patient goals of care.
- 2+ years of experience in home-based complex care, geriatrics, or palliative care as a Registered Nurse or Nurse Practitioner.
- 6+ months of NP experience.
- Current licensure in the state of Arizona.
- Completed an accredited NP program with a national certification in Family, Geriatrics, or Adult.
- Unrestricted driver's license and ability to drive to patient homes.
