Salary: $65,000 - $80,000 per year, based on experience.

Job Description:

• We are seeking a skilled Dietitian Specialist to join our team, responsible for implementing the overall nutrition program for our Elder Service Plan.
• Under the general supervision of the Nutrition Manager, you will assist in ensuring that the nutrition programs are in compliance with regulatory agencies and meet PACE and DPH requirements.
• You will provide guidance and advice to participants and their families regarding nutrition, and participate in interdisciplinary teams (IDT) to ensure seamless care.

Responsibilities:

• Perform and document timely initial and annual nutrition assessments, including diet orders, height and weight, lab results, dietary history, and medical conditions.
• Develop individual nutrition plans and effectively communicate them to participants, IDT, Dietary Aides, family members, and other caregivers.
• Ensure the nutrition plan is implemented by providing participant education materials, one-on-one guidance, and supervising Dietary Aides.
• Recommend and arrange for provision of nutrition supplements and home-delivered meals as needed.
• Assist the Nutrition Manager in ensuring kitchen operations comply with OSHA, DPH, PACE, and CMS regulations.

Required Skills and Qualifications:

• Bachelor's degree in Food and Nutrition or Dietetics; Master's degree preferred.
• Current Registered Dietician licensure in MA required.
• One year of professional experience providing nutrition counseling and assessments.
• Excellent written and oral communication skills.
