Job Overview

We are seeking a skilled and compassionate Social Worker-PACE to join our team at Neighborhood Healthcare.

About the Role:

  • This position is responsible for managing a caseload of participants in the PACE program, providing psychosocial assessments, and developing individualized care plans.
  • The successful candidate will work collaboratively with the interdisciplinary team to optimize the health status and quality of life of participants.
  • This role requires strong communication and interpersonal skills, with the ability to build relationships with participants, family members, and healthcare professionals.

Responsibilities:

  • Conduct initial, semi-annual, unscheduled, and annual assessments with participants and their families.
  • Provide participant health status updates to the interdisciplinary team during daily meetings.
  • Obtain extensive psychosocial histories from participants and/or family members upon admission to the program and intermittently at pre-enrollment.
  • Coordinate, schedule, and facilitate family conferences to address levels of care, medication/treatment non-compliance, out-of-home placement, complex diagnoses, behavioral concerns, and contracts.

Requirements:

  • Masters degree in social work required.
  • Valid BLS certification in accordance with the American Heart Association Guidelines required upon hire.
  • One year experience working with frail or elderly populations required.
  • One year of social work experience preferred.

Additional Qualifications:

  • Excellent verbal and written communication skills, including superior composition, typing, and proofreading skills.
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.

Salary and Benefits:

The salary range for this position is $80,600 - $92,560 annually, depending on experience. We offer a comprehensive benefits package, including partially company-paid medical, dental, and vision plans, two plus weeks of vacation, nine holidays, sick/personal time, volunteer time off, 403b retirement plan, and more.
