Supplemental Health Care is seeking a skilled Registered Nurse to provide high-quality hospice care to patients in their homes. As a Hospice RN, you will work closely with a partnering in-home hospice care provider to deliver compassionate and personalized care to individuals in need.
Responsibilities:
- Provide direct patient care, including administering medications, wound care, and other treatments as needed
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with the interdisciplinary team to ensure seamless care coordination
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements:
- Current Oregon RN License
- Minimum of 1 year recent relevant nursing experience
- Electronic Charting experience
- American Heart Association BLS
- Valid U.S. Driver's License
