Job Summary

We are seeking a skilled Licensed Practical Nurse to join our Hematology/Oncology team as a Patient Navigator. In this role, you will play a vital part in guiding incoming patients through our program, providing education, and ensuring continuity of care.

Key Responsibilities
  • Utilize clinical knowledge to facilitate patient introduction to the program
  • Perform and document patient needs assessments via telephone or review of documentation
  • Collaborate with healthcare providers to coordinate and implement patient care plans
  • Effectively communicate with patients regarding their care
  • Coordinate patient activities, such as support groups and educational series
  • Provide limited patient and family education based on individual needs
  • Maintain systems and processes for timely follow-up and surveillance of patients
  • Serve as a liaison with Scheduling Services to ensure appropriate patient scheduling
  • Develop and implement patient screening tools and efficient clinic procedures
Requirements
  • 2 years of LPN work experience
  • Graduate from a Specialty Training Program in Nursing
  • Licensed Practical Nurse certification
