As a Memory Care Specialist, you will be part of a team that is dedicated to delivering exceptional care and services to our residents. Your role will involve supervising and coordinating medication management, resident care delivery, and activities programs in the Memory Care neighborhood.

Key Responsibilities:

Assist in the initial assessment and development of Individualized Service Plans for incoming residents.
Interview and examine current residents to assess needs, monitor and record resident medical condition and behavioral changes, and recommend adjustments to care plans.
Coordinate the medication management process and supervise the ordering, delivery, storage, administering, and disposal of medications.
Provide direct daily supervision of Medication Assistant staff activities.

Requirements:

Must be licensed as an LPN and have a minimum of three years of experience in a similar position.
Ability to work effectively within a team-based environment and communicate clearly with residents, families, and staff.
Strong time management and organizational skills, with the ability to calculate figures related to budgetary items, staffing needs, and medication dosages.
Knowledge of applicable healthcare and employment Federal and State regulations.
Ability to read and write English, and follow verbal and written instructions.
