UI Health is seeking a highly skilled Medical Social Worker to join our team. As a Medical Social Worker, you will play a critical role in providing comprehensive psychosocial assessments, developing and implementing plans of intervention, and coordinating discharge planning for patients and their families.
Key Responsibilities- Conduct comprehensive psychosocial assessments, including social, emotional, cultural, mental status, environmental, and financial circumstances, in conjunction with an interdisciplinary assessment of the patient.
- Develop and implement plans of intervention based on mutually established goals, addressing reactions to illness, adjustment to the medical setting, and linkage to community resources.
- Coordinate discharge planning, including placement coordination and resource utilization, to ensure a smooth transition for patients and their families.
- Participate in daily Multidisciplinary Rounds (MDRs) and Unit/Clinic based rounds to ensure effective communication and coordination of care.
- Utilize software to cultivate resources mapped directly from the Patient's medical, social, and cultural needs, insurance coverage, financial status, geographical preference, and physician recommendations.
- Escalate to supervisor/director when barriers are present to facilitate resolution and proactively identify and resolve delays and obstacles to discharge.
- Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.
- Current State of IL Licensure as a Licensed Social Worker or a Licensed Clinical Social Worker, or three (3) years of progressively more responsible full-time experience in social work in a medical/clinical or social service agency setting.
