We are seeking a compassionate and skilled Licensed Practical Nurse/Licensed Vocational Nurse to join our Home Health/Hospice team. As a key member of our care team, you will provide high-quality patient care in a variety of settings, including home health and hospice.
Responsibilities- Provide 1:1 care for clients, following their individualized plan of care, while maintaining their dignity and independence.
- Monitor clients' vital statistics and report any abnormal findings to the case manager and physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, and document the results.
- Complete routines established by other care professionals, such as physical therapists, occupational therapists, and speech therapists.
- Ensure infection control policies are maintained at all times, and report any possible communicable diseases or infections.
- Accurately document nursing actions, communicate with family, case manager, and physicians, and document progress and outcomes for established goals.
- Order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
- Monitor incidents involving individuals, complete incident reports, and inform case managers.
- Ensure all medical equipment is maintained and report any faulty equipment to Durable Medical Equipment for repair.
- Proof of current license as an LPN/LVN in good standing from the state(s) of practice.
- Current Cardiopulmonary resuscitation certification.
- Ability to work in a home-like setting.
- Ability to bend, stoop, push, pull, sit, and walk for periods of time.
- Able to utilize proper body mechanics while lifting 50 pounds.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Must have completed post-secondary coursework to facilitate licensure as an LPN/LVN.
