As a Home Health Care Registered Nurse, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

Responsibilities:
  • Deliver high-quality patient care in a home setting, following a designated care plan and collaborating with the healthcare team.
  • Conduct thorough assessments, develop and implement individualized care plans, and provide ongoing monitoring and evaluation of patient progress.
  • Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Document patient information accurately and thoroughly, utilizing a touch pad tablet.
Qualifications:
  • A current license as a Registered Nurse in Rhode Island.
  • One year of prior clinical experience as a licensed RN.
