About the Role:

This is an exciting opportunity for a motivated and compassionate Advanced Practice Clinician to join our team at Optum Senior Community Care.

You will work closely with our clients to provide high-quality, patient-centered care that addresses their physical, emotional, and social needs.

Key Responsibilities:

  • Conduct comprehensive assessments and develop personalized care plans
  • Manage acute medical situations and exacerbations of chronic conditions
  • Provide advanced illness and end-of-life discussions
  • Collaborate with the care team to ensure seamless care coordination

Requirements:

  • Master of Science in Nursing or Doctor of Nursing Practice degree from an accredited program
  • Active and unrestricted license in the state where you reside
  • National board certification as a Nurse Practitioner (e.g., Family Nurse Practitioner, Adult-Gerontology Acute Care Nurse Practitioner)
  • Current DEA licensure/prescriptive authority or ability to obtain post-hire
