Job Summary
Transitional Care Management is seeking a skilled and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at Stephenson Nursing Center. As a member of our nursing staff, you will play a vital role in providing high-quality clinical care to our patients and residents. Responsibilities
As a Registered Nurse or Licensed Practical Nurse, you will be responsible for providing direct patient care, administering medications, and collaborating with our multidisciplinary team to achieve optimal patient outcomes. You will also have the opportunity to participate in our center's quality improvement initiatives and contribute to the development of our nursing policies and procedures. Requirements
To be considered for this role, you must have a current RN or LPN license in the state of Illinois and prior experience working in a skilled nursing facility. Electronic medical records experience is also preferred. We follow the CMS and IDPH rules for the Covid-19 vaccine, and you will be required to be fully vaccinated or submit an exemption.
