About the Role:

We're seeking a highly skilled and experienced Independent Optometrist to take on a leading role in patient care within our Walmart stores. As an Independent Optometrist, you'll be responsible for establishing your days of practice, hours of operation, patient schedule, and fee schedule. You'll also have access to equipment/instrumentation, administrative, and/or Opt-Tech support services (if state allows) for a fair market value.

Requirements:

  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school
  • Valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
