We are seeking a highly skilled Community Health Registered Nurse to join our team at Neighborhood Health Center - WNY. As a Community Health Registered Nurse, you will play a vital role in providing high-quality care to diverse individuals and families, regardless of their ability to pay.
Key Responsibilities- Builds trusting relationships with patients to facilitate access to healthcare services and meet their health needs.
- Engages patients in health promotion, maintenance, and restoration activities.
- Works in partnership with our multidisciplinary team to advocate for patients and ensure their health needs are met.
- Assists patients and families with accessing, navigating, and connecting with internal providers and services, outside community resources and programs, and other services as needed.
- Revises and reprioritizes plan of patient care, as needed; identifies and addresses any barriers affecting a patient's ability to adhere to the plan.
- Conducts community outreach, home visits, shelter/street outreach to identify people who are experiencing homelessness and other social support needs and educates about health needs.
- Graduated from an accredited two-year nursing school.
- Must possess a current NYS Professional Registered Nurse license.
- Must have current CPR certification.
- The ideal candidate has knowledge of case/care management principles and has the ability to work with a diverse group of patients and understands the impact of social determinants on health.
- Competitive hourly rate: $30 - $40 an hour (based on a full-time, 40-hour work week).
- RN Tuition Reimbursement Potential: Our FQHC status provides a unique opportunity for tuition reimbursement for registered nurses, up to 60% repayment of qualified student loans directly related to a nursing degree.
- Sign-on bonus for external hires, paid in installments throughout the employee's successful first year of employment.
