Position Overview

Menorah Park of CNY is seeking a qualified Clinical Social Services Specialist who possesses a strong foundation in psychosocial care. The successful candidate will be adept at assisting individuals in navigating their challenges and guiding families through significant decisions regarding their loved ones.

Key Responsibilities:
  • Deliver personalized counseling and supportive consultations to residents.
  • Manage referrals and case management effectively to provide comprehensive social-emotional support.
  • Establish professional relationships with residents and families to evaluate the necessity for Social Work intervention.
  • Support departmental staff with adherence to Social Services policies and enhance overall resident care.
  • Serve as a mediator in addressing and resolving resident grievances and concerns.
Qualifications:
  • A minimum of 2 years of clinical social work experience is preferred.
  • A Bachelor's Degree in Social Work is mandatory.
  • Proficient in conducting psychosocial assessments and formulating actionable care plans.
  • Exceptional listening and communication abilities.
  • Familiarity with Electronic Medical Records (EMRs) is essential.
  • Knowledge of Department of Health guidelines is advantageous.
  • Willingness to provide after-hours support as required.
  • Experience in Long-Term Care or Skilled Nursing Facilities is preferred.
