Job Summary:

The Personal Care Aide will provide personal care and assistance to participants in their homes, centers, or in the community. This includes bathing, dressing, feeding, toileting, transfers, lifting, and incontinence maintenance, as well as laundry, meal preparation, and clean-up. The ideal candidate will have a strong commitment to providing excellent care and support, and will be able to work independently and as part of a team.

Responsibilities:
  • Provide personal care and assistance to participants, including bathing, dressing, feeding, toileting, transfers, lifting, and incontinence maintenance.
  • Assist with laundry, meal preparation, and clean-up.
  • Travel to and from participants' homes using a properly registered, inspected, and insured vehicle.
  • Accompany participants to appointments as needed.
  • Monitor and prompt medication as appropriate.
  • Report any changes in participants' medical or physical status to the appropriate team leader or medical team members.
  • Document services provided and observations on appropriate forms.
  • Work one-on-one with assigned participants to implement specific activity plans.
  • Provide care to participants per interdisciplinary team plan of care.
  • Prepare participants for treatments within the center clinic.
  • Participate in and support quality improvement initiatives.
Requirements:
  • High school diploma or GED.
  • CPR and First Aid Certification.
  • One year of experience working with the frail and elderly population, or willingness to complete company-paid training.
  • Valid state driver's license with a clean driving record.
  • Successful completion of background check, physical, drug screening, and TB screening.
Physical Requirements:
  • Ability to move intermittently throughout the workday.
  • Ability to lift, push, pull, and move a minimum of 25 pounds.
  • Ability to assist with the evacuation of participants.
